Having worked in chemical plants ran by americans I can seriously tell you there is no single failure or stupidity that could have caused this.
Plants, especially refineries, and especially those on US soil are so incredibly overdesigned and overtrained that to take a piss, you need a permit signed and checked by 3 people, and only if you wear the proper clothing and equipment to enter the toilet area, after which a team inspects what you have done and documents it.
So what remains are geotechnical inluences, aliens, freak off-scale environment circumstance, multiple cascading failures, and yes, towelheads.

It used to, but not any longer. They changed the name of the company to BP in the 1950s IIRC. When BP acquired Amoco a few years ago, they launched a big ad campaign in the US that "suggested" that 'BP' stood for 'Beyond Petroleum.' Implying, I guess, that oil is just a temporary sideline while they're working on some new fusion technology... or something.

Yes, I'm sure BP are very sad about the effect on their profit margin, which, come ot think of it, will probably be negligible, after they've hiked prices claiming a 'shortfall in production due to this terrible accident'
As for the dead workers, well, that's a shame too, right?
