want to go back to my maiden name
want to apply to renew my passport in previous name
not yet divorced

have been seperated for 4 and a half
not go around to getting divorced yet
dont hate husband, dont really have much in common anymore though

i can't help you i'm afraid.
have you tried just googling how to do it then going to (wherever it's done) and asking them?
I'd guess it's done by the registry of birth marriages and deaths, you can probably ring your local one asking for a form then fill it in wand send an administrative fee.

Changing your name is as simple as using a new name, there is no legal procedure. The only caveat is that you must not be doing it to defraud anyone.
A letter from a responsible person (doctor/bank etc) shoudl suffice for a new passport I believe, especially as you are only reverting to your maiden name.

Saying that tho because you're just reverting to your maiden name you might be able to get away with just a stat dec (even with you living in England).
