to be fair, the Guardian is pretty crap but I wouldn't be suprised if some (or all) of those revelations were true, seeing as cops (read: everyone) generally lie their asses off to protect themselves when they do shit like this.
the first offical story you hear in cases like this is generally debunked after a while.

This is speculation on a leaked document.
The IPCC is the proper process through which all of the evidence will be considered.
This leak distorts the picture and though we might not like it, it takes a long time to determine what the truth is. It only takes seconds to create a lie.

If the truth does turn out to be total incompetence, is it possible that the officer who pulled the trigger (and perhaps the superior who gave permission/the order) could find themselves on a murder charge in a public trial?

it wouldn't be murder if it was just incompetence. but yes, there's nothing to prevent such a trial and policemen have been put on trial before
